What is Transitions?
Lower Cape Fear LifeCare Center Transitions Program provides free
supportive services for clients with advanced, life-limiting illness
and their caregivers and families.
What kinds of support are offered?
Support can include information about obtaining needed community
services, encouragement to discuss health care options and decisions
with health providers and family members, respite for caregivers,
help with practical matters such as occasional transportation and
errands, and friendship for clients and their caregivers.
Who provides Transitions services?
A social worker who is an experienced case manager coordinates Transitions
Program. Specially trained, experienced and caring volunteers offer
education, support, friendship and respite to patients and their
families while they live with their illness.
Who qualifies for Transitions?
Patients who are in advanced stages of life-limiting illnesses,
who are not receiving hospice services, and who, with their families,
desire supportive services to help them cope with their many challenges
of advanced illness.
What is the Transitions service area?
Transitions is first being piloted in New Hanover County for a year.
As the program grows, it will expand to include more of the Lower
Cape Fear LifeCareCenter service area.
What is the cost of Transitions?
Transitions is offered at no charge to our community, and Lower
Cape Fear LifeCare Center does not receive reimbursement for these
services. We are grateful for the support received from The Catherine
Kennedy Home and the Cape Fear Memorial foundations and appreciate
donations to the program.
Who can refer a patient to Transitions?
Anyone can call the Transitions coordinator for information or to
make a referral to receive Transitions services. The Transitions
coordinator is available to talk with patients, family members,
caregivers and health care providers about this program. These discussions
determine the appropriate amount of support and services needed
and continue to reevaluate the plan of support as the illness progresses.
