What is Geriatric Care Management?
Geriatric Care Management is a contractual, fee-for-service
program intended to provide services to:
§
Comprehensively assess an elder’s care needs and provide a
care plan(s) based on those needs
§
Teach individuals and their families and/or caregivers how to
assist their elders in obtaining needed health care, financial,
residential care and community services and information
§
Assist elders and their family members to plan for current
and future needs for home care and/or residential care services
§
Help elders and their families make arrangements for needed
services.
§
Explore health care and treatment options so that individuals
and their families can make informed decisions
§
Monitor needs, care, and services
Who can benefit from Geriatric Care Management?
Patients, clients, and families concerned about age-related
limitations and planning for health, emotional, social and
residential needs now and in the future can benefit from the
services offered by Geriatric Care Management.
When is it time to seek Geriatric Care Management?
Often the request for this service is triggered by family
members and friends who notice that an elder’s personality or
habits are changing. These changes can include:

Becoming increasingly suspicious
§
Suddenly losing weight
§
Getting lost while walking or driving
§
Frequently falling
§
Not seeking medical attention for health problems
§
Neglecting to pay bills
