Bereavement Counselor, Columbus/Bladen - 0.5 FTE (20 hours per week) The Bereavement Counselor provides professional grief counseling and education to families of former patients. Group grief education and support is provided to families of former patients and the community. As a member of the IDT, the Bereavement Counselor provides consultation with team members about grief issues and participates in the initial assessment for Bereavement services. The Bereavement Counselor is responsible for continuing his/her personal development to enhance practice. Performs other duties as assigned. Development Coordinator, Wilmington - 1.0 FTE The Development Coordinator is responsible for writing and submitting timely grant application packages in a manner that maintains a funding pipeline that meets or exceeds the fund-raising goals of the agency. Supports Development activities by identifying and researching prospects; preparing program descriptions, solicitation letters and case statements; Primary contact person for Vice President of Development/Board of Trustees and assumes responsibility for administrative projects for Board of Trustees and Vice President of Development. Develops and writes quality marketing materials and presentations ensuring that all communication with donors and prospects effectively conveys the intent of the LCFH Foundation objectives. Performs other duties as assigned. Bachelor’s degree preferred or commensurate experience. Minimum of 3 years experience in development or fundraising required; ability to demonstrate grant/proposal writing skills and results required. Internal Auditor, Corporate 1.0 FTE - The Internal Auditor is primarily responsible for carrying out an independent appraisal activity within the agency for the review of accounting, financial and other operations. Physician – Palliative Care/Hospice, Columbus/Bladen County - 1.0 FTE The Palliative Care/Hospice Physician is responsible for the medical component of the palliative care program, performs consultative services to patients/residents and families with life-limiting illnesses; helps to determine and implement policy concerning the medical component of palliative care program and services; and supervises direct medical care with other members of the palliative care team. Acts in the capacity of hospice medical director overseeing the medical components of care to hospice patients in all settings. Performs other duties as assigned. Graduate of an accredited school of medicine or osteopathy with a degree in medicine or osteopathy required. Current NC practicing physician licensure, valid North Carolina driver’s license; credentialed member of NHHN Medical Staff required. Physician's Assistant - 2 1.0 FTE (40 hours) positions available: 1 for Bladen and Columbus counties and 1 for Pender County - Provides direct patient care for patients with life- limiting illnesses. The patient population can include the family practitioner range of children through geriatric patients (65+ years). Is responsible for using expert skills in history taking, physical examination, initiation and interpretation of tests, analysis of data, and the implementation and evaluation of plans of care. Effectively manages the Plan of Care for each patient based on clinical knowledge, data, patient preference and benefits to the patient. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. The patient population can include adults (45-64 years) through geriatric patients (65+ years). Responsible for using expert skills in history taking, physical examination, initiation and interpretation of tests, analysis of data, and the implementation and evaluation of plans of care. Effectively manages the Plan of Care for each patient based on clinical knowledge, data, patient preference and benefits to the patient. Management of each patient also includes appropriately and effectively delegating patient care duties to other members of the team in order to assure optimal patient outcomes. Works with Palliative Care Center physicians in the hospital, nursing homes and physician’s office and patient’s homes. The Nurse Practitioner is also responsible for continuing his/her own personal development to enhance practice and the profession of Nursing. Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program required. Graduate of a Masters degree in nursing program with a major or post-masters as a nurse practitioner required. Current license to practice professional nursing in the State of North Carolina required. Approval for practice as a nurse practitioner by the North Carolina Board of Nursing and the North Carolina Board of Medical Examiners required.
